Resumen : | Determinar si la terapia de presión negativa es más eficaz que la
terapia convencional en el tratamiento de la úlcera de pie diabético complicada
en adultos.
Material y métodos: Se llevó a cabo una revisión sistemática y metaanálisis, el
proceso de tamizaje permitió obtener 8 ensayos clínicos y 4 estudios
observacionales, para comparar la eficacia de la terapia de presión negativa
versus la terapia convencional.
Resultados: La población de los ensayos clínicos fueron 760 en total, 373 con
la terapia de presión negativa y 387 con la terapia convencional; se obtuvieron
para los desenlaces, tiempo de formación de tejido de granulación (MD = − 1.77,
IC 95% [-3.35 − -0.20], I2 = 97%, p = 0.03), profundidad de la úlcera (MD = 5.87,
IC 95% [- 2.30 – 14.03], I2 = 96%, p = 0.16), amputación (RR = 0,79, IC 95%
[0.56 – 1.11], I2 = 26%, p = 0.17); dolor (RR = 1.41, IC 95% [1.07 – 1.85], I2 =
0%, p = 0.01), infección (RR= 0.50, IC 95% [0.31 – 0.81], I2=58%, p = 0.005),
sangrado (RR=0.88, IC 95% [0.53 – 1.45], p= 0.61), estancia hospitalaria (MD =
− 15.6, IC 95% [- 19.89 − - 11.31], I2=79%, p < 0.00001). La población evaluada
en los estudios observacionales fue 222, con la TPN fue 100 y con la TC 122; se
obtuvieron para los desenlaces, tiempo de formación de tejido de granulación
(MD = − 5.43, IC 95% [- 7.42 − -3.43], I2=0%, p<0.00001), profundidad de la
úlcera (MD= 0.40, IC 95% [- 1.66 – 2.46], p= 0.70), amputación (RR= 0.39, IC
95% [0.16 – 0.95], p=0.04), dolor (RR=7.42, IC 95% [0.37 −149.73], p=0.19),
infección (RR=0.39, IC 95% [0.16 – 0.95], p=0.04) y estancia hospitalaria (MD=
- 10.90, IC 95% [ - 15.21 − - 6.59], p < 0.00001).
Conclusiones: La TPN demostró tener más eficacia que la TC al disminuir el

Conclusions: The NPWT showed have more efficacious that CT to reduce the time of formation of granulation tissue, the hospital stay, deep of ulcer, and the risk of secondary amputations, infections and bleeding; in addiction of give better pain relief and quality life. |
