Resumen : | Identificar la asociación entre el retraso quirúrgico y el desarrollo de
complicaciones en pacientes con apendicitis aguda atendidos en el Hospital de
Apoyo II-2 Sullana.
Métodos: Estudio de tipo transversal y analítico. Se incluyo a 80 pacientes en
diagnóstico de apendicitis aguda por emergencia de cirugía, los cuales fueron
seleccionados de forma aleatoria entre los pacientes atendidos entre enero a
junio del 2021. El retraso quirúrgico se evaluó como una medida de tiempo
compuesta del tiempo sintomático prehospitalario, tiempo hospitalario
prequirúrgico y tiempo sintomático total, y las complicaciones se identificaron
según historia clínica hasta el séptimo día postoperatorio. Para cuantificar la
fuerza de asociación se calculó razones de prevalencia (RP) e intervalo de
confianza (IC95%).
Resultados: El 38.7% de los participantes del estudio presentaron
complicaciones postoperatorias. Los pacientes con complicaciones tuvieron en
promedio 34.61 años y mayor proporción de sexo masculino (58.08%). En el
análisis multivariado se identificó como factores asociados a complicaciones
postoperatorias las medidas de retraso quirúrgico propuestas [tiempo
sintomático prehospitalario (RP=1.18, IC95%: 1.12 - 1.23, p<0.001), el tiempo
hospitalario prequirúrgico (RP=1.18, IC95%: 1.12 - 1.23, p<0.001) y tiempo
sintomático total (RP=1.14, IC95%: 1.10 - 1.18, p<0.001)].
Conclusiones: El retraso quirúrgico, ya sea por demora en acudir a un centro
de atención media o dilatación del tiempo para el ingreso a sala de operaciones,
se asociaron con una mayor prevalecía de complicaciones postoperatorias. To identify the association between surgical delay and the development of complications in patients with acute appendicitis treated at the Hospital de Apoyo II-2 Sullana. Methods: Cross-sectional and analytical study. We included 80 patients diagnosed with acute appendicitis by emergency surgery, who were randomly selected among the patients attended between January and June 2021. Surgical delay was evaluated as a time measure composed of pre-hospital symptomatic time, pre-surgical hospital time and total symptomatic time, and complications were identified according to clinical history up to the seventh postoperative day. To quantify the strength of association, prevalence ratios (PR) and confidence interval (CI95%) were calculated. Results: 38.7% of the study participants presented postoperative complications. Patients with complications were 34.61 years old on average and a higher proportion were male (58.08%). In the multivariate analysis, the proposed surgical delay measures [pre-hospital symptomatic time (PR=1.18, 95%CI: 1.12 - 1.23, p<0.001), pre-surgical hospital time (PR=1.18, 95%CI: 1.12 - 1.23, p<0.001) and total symptomatic time (PR=1.14, 95%CI: 1.10 - 1.18, p<0.001)] were identified as factors associated with postoperative complications. Conclusions: Surgical delay, either due to delay in going to a middle care center or dilation of the time for admission to the operating room, were associated with a higher prevalence of postoperative complications |
