Resumen : | Demostrar si la hiperglucemia preoperatoria es factor pronóstico de
morbilidad postoperatoria en pacientes gastrectomizados por cáncer gástrico en
el Instituto Regional de Enfermedades Neoplásicas - Trujillo.
Material y métodos: Se llevó a cabo un estudio analítico, de casos y controles
retrospectivo en el que se incluyeron a 144 pacientes con diagnóstico de cáncer
gástrico, según criterios de selección los cuales se dividieron en función de la
presencia o no de morbilidad postoperatoria, se empleó la prueba estadística chi
cuadrado y la prueba estadística t de student para verificar la significancia de la
diferencia entre las frecuencias de las variables cualitativas y cuantitativas
respectivamente. El estadígrafo realizó la determinación del odds ratio y de su
correspondiente intervalo de confianza de 95%.
Resultados: El promedio de estancia hospitalaria y la frecuencia de
hipoalbuminemia, anemia y razón neutrófilo linfocito elevada fueron
significativamente mayores en el grupo de gastrectomizados con morbilidad
postoperatoria (p<0.05); la proporción de hiperglucemia preoperatoria en
pacientes gastrectomizados por cáncer gástrico con morbilidad postoperatoria
fue de 25%; y en los sin morbilidad postoperatoria ésta fue de 8%. La
hiperglucemia preoperatoria es factor pronóstico de morbilidad postoperatoria en
pacientes gastrectomizados por cáncer gástrico con un odds ratio de 3.67 el cual
fue significativo (p<0.05). En el análisis multivariado se reconoce a la
hipoalbuminemia, la anemia, la razón neutrófilo linfocito elevada y la
hiperglucemia preoperatoria como factores pronósticos de morbilidad
postoperatoria en pacientes con cáncer gástrico gastrectomizados.
Conclusión: La hiperglucemia preoperatoria es factor pronóstico de morbilidad
postoperatoria en pacientes gastrectomizados por cáncer gástrico en el Instituto
Regional de Enfermedades Neoplásicas. To demonstrate whether preoperative hyperglycemia is a prognostic factor for postoperative morbidity in gastrectomy patients for gastric cancer at the Regional Institute of Neoplastic Diseases-Trujillo. Material and methods: An analytical, retrospective case-control study was carried out on 144 patients diagnosed with gastric cancer, according to selection criteria which were divided according to the presence or absence of postoperative morbidity, Chi square and the t – student, were applied in order to verify the meaning of difference between frequencies of quality and quantitative variables. The statistician determined odds ratio and its corresponding confidence Interval was 95%. Results: The average of hospital staying and the frequency of hypoalbuminemia, anemia and elevated neutrophil lymphocyte ratio were significantly higher in the gastrectomy group with postoperative morbidity (p<0.05); the proportion of preoperative hyperglycemia in gastrectomy patients because of gastric cancer with postoperative morbidity was 25%; and in non-postoperative morbidity was 8%. Preoperative hyperglycemia is a prognostic factor of postoperative morbidity in patients’ gastrectomy because of gastric cancer with an odds ratio of 3.67, which was significant (p<0.05); In multivariate analysis, hypoalbuminemia, anemia, elevated neutrophil-lymphocyte ratio, and preoperative hyperglycemia are recognized as prognostic factors of postoperative morbidity in gastrectomy patients with gastric cancer. Conclusion: Preoperative hyperglycemia is a prognostic factor of postoperative morbidity in gastrectomy patients because of gastric cancer at the Regional Institute of Neoplastic Diseases. |
