Resumen : | Analizar los factores asociados a infección por Acinetobacter baumannii
multidrogorresistente (Ab-MDR) en pacientes de la unidad de cuidados intensivos
(UCI) de los hospitales: Hospital Regional Docente De Trujillo, Hospital Belén De
Trujillo y Hospital De Alta complejidad Virgen De La Puerta, 2015 - 2018. Método:
Estudio de casos y controles en donde se revisaron 60 historias clínicas de
pacientes que estuvieron internados en UCI de tres hospitales principales,
divididos según la presencia o no de infección por A. baumannii
multidrogorresistente (30 pacientes por grupo). Se analizaron los factores
asociados mediante el cálculo del odds ratio y el análisis multivariado.
Resultados: La edad promedio fue de 60.3 años en los pacientes con Ab-MDR y
de 46.3 años en los no MDR, tener más de 75 años se asoció significativamente a
Ab-MDR, la estancia prolongada en UCI se presentó en el 90% de los pacientes
con Ab-MDR (OR:13.5, p<0.001), la hipoalbuminemia predominó en el 86.7% de
los pacientes con infección MDR (OR: 4.33, p=0.041), el 83.3% de pacientes con
Ab-MDR estuvo expuesto a catéter venoso central (CVC) (OR:3.82; p=0.049), así
mismo, todos los pacientes presentaron antibioticoterapia previa. El análisis
multivariado mostró que la estancia prolongada en UCI (p=0.035) y el uso de CVC
(p=0.008), se asociaron independientemente a infección por Ab-MDR.
Conclusión: la estancia prolongada en UCI y la utilización de CVC son factores
asociados a infección por A. baumannii multidrogorresistente To analyze the factors associated with multidrug-resistant Acinetobacter baumannii infection (Ab-MDR), treated in the intensive care unit (ICU) of the Hospitals: Hospital Regional Docente De Trujillo, Hospital Belén De Trujillo and Hospital De Alta Complejidad Virgen De La Puerta, 2015 - 2018. Method: Case-control study in which 60 medical records of ICU patients from three main hospitals were reviewed, divided according to the presence or absence of multidrug-resistant A. baumannii infection (30 patients per group). Associated factors were analyzed by calculating the odds ratio and multivariate analysis. Results: The mean age was 60.3 years in the patients with MDR-Ab and 46.3 years in the non-MDR patients, being over 75 years old was significantly associated with MDR-Ab, and prolonged stay in the ICU was present in 90% of the patients. with Ab-MDR (OR: 13.5, p<0.001), hypoalbuminemia predominated in 86.7% of patients with MDR infection (OR: 4.33, p=0.041), 83.3% of patients with Ab-MDR were exposed to central venous catheter (CVC) (OR: 3.82; p=0.049), likewise, all patients had previous antibiotic therapy. The multivariate analysis showed that prolonged stay in the ICU (p = 0.035) and the use of CVC (p = 0.008) were independently associated with Ab-MDR infection. Conclusion: prolonged stay in the ICU and the utilization of CVCs are factors associated with multidrug resistant A. baumannii infection. |
