Resumen : | Demostrar que la escala PANC 3 desarrollada por especialistas de la
Escuela de Medicina de Harvard, en la cual se evalúan 3 parámetros; derrame
pleural, hematocrito >44% e IMC >30 kg/m2
, tiene utilidad como predictor de
severidad en pacientes con pancreatitis aguda.
Materiales y métodos: Estudio retrospectivo, observacional de tipo prueba
diagnóstica, realizado en el servicio de Gastroenterología, del Hospital Regional
Docente de Trujillo que incluyó un total de 160 pacientes con diagnóstico de
pancreatitis aguda. Se calculó la sensibilidad, especificidad, VPP y VPN de la
escala PANC 3 como predictor de severidad en pancreatitis aguda. Además se
calculó la sensibilidad y especificidad de cada uno de los parámetros que
conforman la escala PANC 3 y se determinó que variable interviniente se asoció
a severidad en pancreatitis aguda.
Resultados: La escala PANC 3 presentó una sensibilidad del 73,08%, una
especificidad del 100%, un valor predictivo positivo del 100% y un valor predictivo
negativo del 95,04%. El derrame pleural demostró ser el mejor parámetro para
predecir severidad en pacientes con pancreatitis aguda, con una sensibilidad del
88,46% y una especificidad del 89,55%. El IMC >30kg/m2 presentó una
sensibilidad del 76.92% y una especificidad del 81.34%. El hematocrito >44%
presentó una sensibilidad del 76.92% y una especificidad del 88.06%. La variable
sexo se asoció a pancreatitis aguda severa por tener un valor de p 0.004, siendo
así el sexo masculino el más afectado por presentar un análisis de residuo
estandarizado positivo. El consumo de alcohol y la edad no mostraron una
relación estadísticamente significativa con severidad en pancreatitis aguda.
Conclusiones: La escala PANC 3 tiene utilidad como predictor de severidad en
pacientes con pancreatitis aguda, debido a que se encontró una adecuada
sensibilidad y altos valores de especificidad, VPP y VPN, por lo que
recomendamos implementarla como herramienta en la práctica clínica diaria
para valoración inicial de esta patología. To demonstrate that the PANC 3 score developed by Harvard Medical School specialists; which evaluates 3 parameters: pleural efussion, hematocrit >44% and BMI >30kg/m2 , is useful as a predictor of severity in patients with acute pancreatitis. Materials and methods: Retrospective, observational study of diagnostic test type, carried out in the Gastroenterology service at the “Hospital Regional Docente de Trujillo” which included a total of 160 patients with acute pancreatitis. Sensitivity, specificity, PPV and NPV of the PANC 3 score were calculated as a predictor of severity in acute pancreatitis. We also calculated the sensitivity and specificity of each of the parameters that make up the PANC 3 score and determined which intervening variable was associated with severity in acute pancreatitis. Results: The PANC 3 score showed a sensitivity of 73.08%, a specificity of 100%, a positive predictive value of 100% and a negative predictive value of 95.04%. Pleural effusion proved to be the best parameter predicting severity in patients with acute pancreatitis, with a sensitivity of 88.46% and a specificity of 89.55%. BMI>30kg/m2 presented a sensitivity of 76.92% and a specificity of 81.34%. Hematocrit >44% presented a sensitivity of 76.92% and a specificity of 88.06%. The variable gender was associated with severe acute pancreatitis by having a value of p 0.004, with the male gender being the most affected by presenting a positive standardized residual analysis. Alcohol consumption and age did not show a statistically significant relationship with severity in acute pancreatitis. Conclusions: The PANC 3 score can be used as a predictor of severity in patients with acute pancreatitis, because adequate sensitivity and high specificity, PPV and NPV were found, therefore we recommend implementing it as a tool in daily clinical practice for the initial assessment of this pathology |
