Resumen : | Objetivo: Determinar si la escala del Modelo de Enfermedad Hepática Terminal
(MELD) integrado a lactato arterial tiene validez como predictor de mortalidad
en pacientes con cirrosis hepática del Hospital Belén de Trujillo durante el
periodo 2015 – 2018.
Material y métodos: Estudio retrospectivo seccional de pruebas diagnósticas,
ingresaron pacientes con cirrosis hepática atendidos en el Servicio de
Gastroenterología del Hospital Belén de Trujillo en el período 2015 – 2018 que
cumplieron los criterios de selección (184 pacientes). Seleccionamos por
muestreo aleatorio simple a los pacientes con cirrosis hepática en función de la
presencia (39) o ausencia (145) de mortalidad intrahospitalaria, los asociamos
a las variables intervinentes, calculamos MELD intregrado a lactato arterial para
cada paciente, completamos el tamaño muestral. Se excluye pacientes que
presentaron patologías subyacentes que pudieran influir en la aparición de
mortalidad, se utilizó el cálculo del tamaño para la población infinita. Se utilizó
chi cuadrado para las variables cualitativas; y T de student para las variables
cuantitativas.
Resultados: La frecuencia de hiponatremia, estadio Child Pugh c, INR elevado
e injuria renal aguda fueron significativamente mayores en el grupo de
pacientes fallecidos (p<0.05). La sensibilidad y especificidad fue de 85% y 84%
respectivamente. El valor predictivo positivo y negativo fue de 60% y 95%
respectivamente. La exactitud pronostica fue de 74%.
Conclusión: La escala MELD integrado a lactato arterial tiene validez como
predictor de mortalidad en pacientes con cirrosis hepática del Hospital Belén de
Trujillo durante el periodo 2015 – 2018. Objective: To determine if the scale of the Model of Terminal Liver Disease (MELD integrated to arterial lactate has validity as a predictor of mortality in patients with liver cirrosis of the Hospital Belen of Trujillo during the period 2015 – 2018. Material and methods: Retrospective sectional study of diagnostic tests, patiens with liver cirrhosis treated at the Gastroenterology Service of the Hospital Belen of Trujillo in the period 2015 – 2018 who met the selection criteria (184 patients) were admitted. We selected patients with liver cirrhosis based on the presence (39) or absence (145) of in-hospital mortality by simple random sampling, we associated them with the intervening variables, we calculated MELD integrated with arterial lactate for each patient, we completed the sample size. Patients with underlying pathologies that could influence the onset of mortality are excluded, the calculation of size for the infinite population was used. Chi square was used for the qualitative variables; and student T for the quantitative variables. Results: The frequency of hiponatremia, Child Pugh stage c, elevated INR and acute kidney injury were significantly higher in the group of deceased patients (p<0.05). The Sensitivity and Specificity were 85% and 84% respectively. The Positive and Negative Predictive Value was 60% and 95% respectively. Prognostic Accuracy was 74%. Conclusion: The MELD scale integrated to arterial lactate has validity as a predictor of mortality in patients with liver cirrhosis at Hospital Belen of Trujillo during the period 2015 – 2018. |
