Resumen : | Objetivo: Determinar si el modelo de enfermedad hepática terminal
integrado a colesterol total es factor asociado a mortalidad en
pacientes cirróticos del Hospital Belén de Trujillo.
Material y métodos: Se llevó a cabo un estudio analítico,
observacional, seccional, transversal en el que se incluyeron a 128
pacientes con cirrosis hepática descompensada, según criterios de
selección los cuales se dividieron en 2 grupos: 31 pacientes
fallecidos Y 97 sobrevivientes; aplicándose el odds ratio, y la prueba
estadística chi cuadrado.
Resultados: La frecuencia de hiponatremia y varices esofágicas
fueron significativamente mayores en el grupo de pacientes fallecidos
que en el de sobrevivientes (p<0.05). El promedio de tiempo de
enfermedad, bilirrubina y colesterol total sérico fueron
significativamente diferentes entre el grupo de fallecidos o
sobrevivientes (p<0.05). El puntaje elevado del modelo de
enfermedad hepática terminal integrado a colesterol total es factor
asociado a mortalidad en cirróticos con un odds ratio de 6.1 el cual
fue significativo (p<0.05). En el análisis multivariado se corrobora la
significancia del riesgo para los variables: MELD colesterol elevado,
tiempo de enfermedad, hiponatremia, varices esofágicas como
factores asociados a mortalidad en cirrosis hepática (p<0.05).
Conclusión: El modelo de enfermedad hepática terminal integrado a
colesterol total es factor asociado a mortalidad en pacientes
cirróticos del Hospital Belén de Trujillo. Objective: To determine if the model of terminal liver disease integrated to total cholesterol is a factor associated with mortality in cirrhotic patients at Belén the Trujillo Hospital. Material and methods: An Analytical, observational, sectional, crosssectional study was carried out in which 128 patients with decompensated liver cirrhosis were included, according to selection criteria which were divided into 2 groups: 31 death patients and 97 survivors; applying the odds ratio, and the chi-square statistical test. Results: The frequency of hyponatremia and esophageal varices were significantly higher in the group of deceased patients than in the group of survivors (p <0.05). The average time of illness, bilirubin and serum total cholesterol were significantly different among the group of deceased or survivors (p <0.05). The high score of the terminal liver disease model integrated to total cholesterol is a factor associated with mortality in cirrhotics with an odds ratio of 6.1 which was significant (p <0.05). The multivariate analysis corroborates the significance of the risk for the variables: MELD high cholesterol, disease time, hyponatremia, esophageal varices as factors associated with mortality in liver cirrhosis (p <0.05). Conclusion: The model of terminal liver disease integrated to total cholesterol is a factor associated with mortality in cirrhotic patients at Belén de Trujillo Hospital. |
