Resumen : | Introducción: La neumonía adquirida en la comunidad (NAC) es la enfermedad
respiratoria infecciosa con mayor tasa de mortalidad a nivel mundial, poder
identificar los marcadores pronósticos ayudaría en el manejo preventivo de la
muerte. Objetivo: demostrar que el Índice proteína C-reactiva/albumina es
marcador pronóstico de mortalidad en neumonía adquirida en la comunidad.
Método: Estudio de pruebas diagnósticas en donde se revisaron 126 historias
clínicas de pacientes con NAC, hospitalizados en el Hospital Belén de Trujillo, 2014-
2018, evaluándose el índice PCR/albúmina (IPA) al ingreso y la muerte del paciente
dentro de los 30 días de hospitalización. Se determinó el área bajo la curva ROC
del mejor punto de corte y se calculó el OR ajustado por análisis multivariado
estableciendo el riesgo de muerte de forma independiente. Resultados: El punto
de corte óptimo de IPA fue de 2.5, con área bajo la curva de 0.891 (IC95%: 0.838-
0.944), sensibilidad de 87%, especificidad del 81%, VPP de 67% y VPN del 93%,
así mismo fue un marcador independiente de muerte por NAC incrementando dicho
riesgo en 10.67, superior que el PCR solo (ORa: 7.15). Conclusión: el Índice
proteína C-reactiva/albumina es un marcador mortalidad en NAC. Introduction: Community-acquired pneumonia (CAP) is the infectious respiratory disease with the highest mortality rate worldwide, being able to identify prognostic markers would help in the preventive management of death. Objective: to demonstrate that the C-reactive protein / albumin index is a prognostic marker of mortality in community-acquired pneumonia. Method: Study of diagnostic tests in which 126 clinical records of patients with CAP, hospitalized in the Hospital Belen de Trujillo, 2014-2018, were reviewed, evaluating the PCR / albumin index (IPA) upon admission and the death of the patient within 30 days of hospitalization. The area under the ROC curve of the best cut-off point was determined and the adjusted OR was calculated by multivariate analysis establishing the risk of death independently. Results: The optimal IPA cutoff point was 2.5, with an area under the curve of 0.891 (95% CI: 0.838-0.944), sensitivity of 87%, specificity of 81%, PPV of 67% and NPV of 93%, as well as It was an independent marker of death by NAC, increasing this risk by 10.67, higher than CRP alone (ORa: 7.15). Conclusion: the C-reactive protein / albumin index is a marker of mortality in NAC. |
