Resumen : | Objetivo: Demostrar que el índice nutricional pronóstico <41 es factor
predictor para complicaciones en pancreatitis aguda moderadamente grave
en el Hospital Regional Docente de Trujillo durante el periodo 2014 – 2018.
Material y métodos: Se llevó a cabo un estudio retrospectivo, transversal en
el que se incluyeron a 165 pacientes con pancreatitis aguda moderadamente
grave, los cuales se dividieron en 2 grupos: pacientes con complicaciones
con Indice Nutrcional Pronóstico <41 o ≥41 los cuales se comparon con los
pacientes sin complicaciones en donde se halló el mismo indice;
aplicándose el odds ratio, y la prueba estadística chi cuadrado.
Resultados: Se encontraron diferencias significativas entre los promedios
de edad y puntaje de la escala APACHE II y respecto a las frecuencias de
etiología, diabetes mellitus, hipertensión arterial, obesidad,
hipercolesterolemia y puntaje BISAP entre pacientes con pancreatitis aguda
moderadamente grave con o sin complicaciones locales (necrosis,
infección, absceso pancreático, seudoquiste pancreático y hemorragia
peripancreática). El índice nutricional pronóstico <41 es factor predictor de
complicaciones en pacientes con pancreatitis aguda moderadamente grave
con un odds ratio de 40.9 el cual fue significativo (p<0.05). Las principales
complicaciones locales en pacientes con pancreatitis aguda
moderadamente grave fueron infección, absceso pancreático y
pseudoquiste pancreático. En el análisis multivariado los factores
predictores de complicaciones en pacientes con pancreatitis aguda
moderadamente grave fueron el índice nutricional pronóstico <41, el tener
edad promedio de 67,6 ± 16,5 y el puntaje BISAP mayor o igual a 3 puntos.
Conclusión: El índice nutricional pronóstico <41 es factor predictor para
complicaciones en pancreatitis aguda moderadamente grave en el Hospital
Regional Docente de Trujillo durante el periodo 2014 – 2018. Objective: To demonstrate that the prognostic nutritional index <41 is a predictor for complications in moderately severe acute pancreatitis at the Trujillo Regional Teaching Hospital during the 2014-2018 period. Material and methods: A retrospective, cross-sectional study was conducted in which 165 patients with moderately severe acute pancreatitis were included, which were divided into 2 groups: patients with complications with Nutritional Prognosis Index <41 or ≥41 which were compared with patients without complications where the same index was found; applying the odds ratio, and the chi-square statistical test. Results: Significant differences were found between the age and average scores of the APACHE II scale and regarding the frequencies of etiology, diabetes mellitus, arterial hypertension, obesity, hypercholesterolemia and BISAP score among patients with moderately severe acute pancreatitis with or without local complications (necrosis, infection, pancreatic abscess, pancreatic pseudocyst and peripancreatic hemorrhage). Prognostic nutritional index <41 is a predictor of complications in patients with moderately severe acute pancreatitis with an odds ratio of 40.9 which was significant (p <0.05). The main local complications in patients with moderately severe acute pancreatitis were infection, pancreatic abscess and pancreatic pseudocyst. In the multivariate analysis, the predictive factors for complications in patients with moderately severe acute pancreatitis were the prognostic nutritional index <41, having an average age of 67.6 ± 16.5 and the BISAP score greater than or equal to 3 points. Conclusion: The prognostic nutritional index <41 is a predictor for complications in moderately severe acute pancreatitis at the Trujillo Regional Teaching Hospital during the 2014-2018 period. |
