Resumen : | Demostrar que la escala MELD-NA tiene mayor validez diagnóstica que
el ICC como predictor de mortalidad en pacientes cirróticos hospitalizados en el
servicio de gastroenterología del Hospital Regional Docente De Trujillo, 2016-2019.
Material y métodos: Se llevó a cabo un estudio observacional, de cohorte
retrospectiva y de pruebas diagnósticas en el que se incluyeron a 136 fichas de
registro de pacientes con cirrosis hepática; Se realizó el cálculo de sensibilidad,
especificidad, valor predictivo positivo y negativo, y exactitud diagnóstica de los
scores MELD-NA e ICC.
Resultados: De un total de 135 pacientes incluidos en el estudio, la edad promedio
de los pacientes fallecidos fue de 67,7 y sobrevivientes fue de 65,7 años, con
predominio del género femenino (53.6%), sobre todo en la cirrosis ocasionada por
alcohol (53.6%); fallecieron un total de 25 (18.5%) pacientes y sobrevivieron 110
pacientes (81.5%). La HDA y ascitis, tuvieron una mayor incidencia en los fallecidos
con un 56% y 28%, respectivamente; mientras que en el grupo de sobrevivientes un
38.2% no desarrolló complicación alguna. El punto de corte para el score MELD-NA
fue ≥26 (S: 92%, E: 87%, VPP: 62%, VPN: 98%); y para el ICC fue ≥6 (S: 92%, E:
90%, VPP: 68%, VPN: 98%). El área bajo la curva de la escala MELD-NA e ICC
como predictor de muerte en pacientes con cirrosis hepática fue de 0.85 y 0.65,
respectivamente, siendo esta diferencia estadísticamente significativa (p<0.04).
Conclusión: El score MELD-NA demostró tener una mayor validez pronóstica
frente al ICC como predictor de muerte en pacientes adultos con cirrosis hepática. To demonstrate that the MELD-NA scale has greater diagnostic validity than the ICC as a predictor of mortality in cirrhotic patients hospitalized in the gastroenterology service of the Trujillo Regional Teaching Hospital, 2016-2019. Materials and methods: An observational, retrospective cohort study and diagnostic tests was carried out, in which 136 registry cards of patients with liver cirrhosis were included; The sensitivity, specificity, positive and negative predictive value, and diagnostic accuracy of the MELD-NA and ICC scores were calculated. Results: Out of a total of 136 patients included in the study, the average age of deceased patients was 67.7 and survivors was 65.7 years, with a predominance of the female gender (53.6%), especially in cirrhosis caused by by alcohol (53.6%); A total of 25 (18.5%) patients died and 110 patients (81.5%) survived. HDA and ascites had a higher incidence in the deceased with 56% and 28%, respectively; while in the group of survivors, 38.2% did not develop any complication. The cut-off point for the MELD-Na score was ≥26 (S: 92%, E: 87%, PPV: 62%, NPV: 98%); and for the ICC it was ≥6 (S: 92%, E: 90%, PPV: 68%, NPV: 98%). The area under the curve of the MELD-NA and ICC scale as a predictor of death in patients with liver cirrhosis was 0.85 and 0.65, respectively, this difference being statistically significant (p<0.04). Conclusions: The MELD-NA score shown to have a greater prognostic validity than to ICC as a predictor of death in adult patients with liver cirrhosis. |
