Resumen : | Determinar la diferencia en los desenlaces clínicos entre infección
urinaria por gérmenes BLEE y no BLEE en un hospital base de Trujillo.
Material y métodos: Se llevó a cabo un estudio de cohortes retrospectivas
en 94 pacientes con infección urinaria, los cuales se dividieron en función de
la presencia de gérmenes BLEE y no BLEE. Se comparó el desenlace clínico
a través del cálculo del Chi cuadrado y el riesgo relativo, considerando un
valor significativo cuando p< 0,05.
Resultados: En el grupo de pacientes con infección por gérmenes BLEE se
encontró que el género femenino fue significativamente mayor (p<0,05). La
mortalidad intrahospitalaria en pacientes con infección urinaria por BLEE se
asoció a anemia (p=0.034). La estancia hospitalaria prolongada en pacientes
con infección urinaria se asoció a infección por gérmenes BLEE (75,0%
versus 40,3%; p = 0,001; RR 1,31; IC95% 0,81-2,11) y con la mayor edad
promedio de los pacientes (68,6 ± 15,7 versus 61,8 ± 17,5 años; p = 0,048).
Al realizarse el análisis multivariado de estos se encontró que solo la
presencia de ITU por gérmenes BLEE fue más significativa (p = 0,034; RR
1,832; IC 95% 1,046-3,210). No se encontró asociación significa en el ingreso
en la unidad de cuidados intensivos (UCI).
Conclusión: La infección del tracto urinario por gérmenes BLEE estuvo
asociada al género femenino, además la mortalidad intrahospitalaria en
pacientes con infección por gérmenes BLEE se asoció a anemia, la estancia
hospitalaria prolongada es mas frecuente en pacientes con infección por
gérmenes BLEE y en la edad >68 años To determine the difference in clinical outcomes between urinary infection by ESBL and non-ESBL germs in a Trujillo Hospital. Material and methods: A retrospective cohort study was carried out in 93 patients with urinary tract infection, which were divided according to the presence of ESBL germs or not. The clinical outcome was compared through the chi-square calculation and the relative risk, considering a significant value when p was <0.05. Results: In the group of patients with ESBL infection, it was found that the female gender was significantly higher (p<0.05). In-hospital mortality in patients with ESBL urinary tract infection was associated with anemia (p=0.034). Prolonged hospital stay in patients with urinary tract infection was associated with infection by ESBL germs (75.0% versus 40.3%; p = 0.001; RR 1.31; 95% CI 0.81-2.11) and with the greater mean age of patients (68.6 ± 15.7 versus 61.8 ± 17.5 years; p = 0.048). When performing the multivariate analysis of these, it was found that only the presence of UTI due to ESBL germs was more significant (p = 0.034; RR 1.832; 95% CI 1.046-3.210). No significant association was found on admission to the intensive care unit (ICU). Conclusion: Urinary tract infection by ESBL germs was associated with the female gender, in addition, in-hospital mortality in patients with ESBL infection was associated with anemia, prolonged hospital stay is more frequent in patients with ESBL infection and in older age. >68 years |
