Resumen : | Determinar el rendimiento diagnóstico del índice volumen
plaquetario medio/linfocitos elevado en la detección de nefropatía
diabética
Materiales y métodos: Se analizaron las historias clínicas de 176
pacientes con diabetes mellitus tipo 2 atendidos en el hospital Víctor
Lazarte Echegaray de Trujillo entre los años 2018 a 2020. Se determinó
la validez diagnóstica del índice volumen plaquetario medio/linfocitos
elevado (≥ 3,60) comparado a albuminuria en orina de 24 horas o. tasa
albuminuria/creatinuria ≥ 30 mg/g en el diagnóstico de nefropatía
diabética. Los datos se procesaron en Excel y se analizaron en EPIDAT
4.2 y SPSS Statistics 26.
Resultados: El índice volumen plaquetario medio/linfocitos elevado en
el diagnóstico de nefropatía diabética tuvo una sensibilidad de 91,0%,
una especificidad de 42,6%, un área bajo la curva de 0, 729, un valor
predictivo positivo de 78,2% y un valor predictivo negativo de 67,6%.
Hubieron 93 (76,2%) pacientes ≥ de 60 años en el grupo con nefropatía
diabética y 28 (51,9%) en el grupo sin nefropatía (p= 0,001). Pacientes
con un tiempo de enfermedad diabética ≥ de 10 años hubieron 108
(88,5%) en el grupo con nefropatía diabética y 24 (44,4%) en el grupo
sin nefropatía (p<0,001)
Conclusiones: El índice volumen plaquetario medio/linfocitos elevado
tiene un adecuado rendimiento diagnóstico en la detección de
nefropatía diabética. To determine the diagnostic performance of the elevated mean platelet volume/lymphocyte index in the detection of diabetic nephropathy. Materials and Methods: The medical records of 176 patients with type 2 diabetes mellitus seen at the Victor Lazarte Echegaray hospital in Trujillo between 2018 and 2020 were analyzed. The diagnostic validity of the elevated mean platelet volume/lymphocyte index (≥3.60) compared to albuminuria/creatinuria ≥ 30 mg/g in the diagnosis of diabetic nephropathy was determined. Data were processed in Excel and analyzed in EPIDAT 4.2 and SPSS Statistics 26. Results: The elevated mean platelet volume/ lymphocyte index in the diagnosis of diabetic nephropathy had a sensitivity of 91.0%, specificity of 42.6%, an area under the curve of 0.729, a positive predictive value of 78.2% and a positive predictive value of 67.6%. Patients ≥ 60 years of age were 93 (76.2%) in the group with diabetic nephropathy and 28 (51.9%) in the group without nephropathy (p= 0.001). Patients with a time of diabetic disease ≥ 10 years there were 108 (88.5%) in the group with diabetic nephropathy and 24 (44.4%) in the group without nephropathy (p<0.001).
