Resumen : | Objetivo: Evaluar si la hipoalbuminemia es predictor de pancreatitis aguda
severa.
Material y método: Se realizó un estudio analítico, observacional, cohorte
retrospectiva; en el que se incluyeron a 152 historias clínicas de pacientes con
diagnóstico de pancreatitis aguda, según los criterios de selección se dividieron
en función de la presencia o no de severidad, se calculó con la prueba estadística
Chi cuadrado y el estadígrafo riesgo relativo. Todas las historias clínicas
incluidas en la investigación contenían los criterios de inclusión y pertenecían al
servicio de Gastroenterología del Hospital Regional Docente de Trujillo.
Resultados: En un total de 152 pacientes, se obtuvo que la frecuencia de
severidad en pacientes con pancreatitis aguda e hipoalbuminemia fue del 33%;
además, la frecuencia de severidad en pacientes con pancreatitis aguda sin
hipoalbuminemia fue del 1%. Determinamos que la hipoalbuminemia es un
predictor de severidad en pacientes con pancreatitis aguda (RR: 25, IC 95% 3.6-
45.4, p <0.05). Así mismo, dentro de las características generales de los
pacientes con pancreatitis aguda, se determinó que la condición de edad
avanzada (OR 31.4, IC 95% 2.5-54.7, p = 0.022), diabetes mellitus (OR: 31.4, IC
95% 1.6-15.8, p = 0.027), proteína C reactiva elevada (OR: 3.89, IC 95% 1.5-7.6,
p = 0.016) y hemoconcentración (OR: 147, IC 95% 4-213.1, p = 0.001), fueron
significativamente relevantes en el grupo de pacientes con pancreatitis aguda
severa.
Conclusión: La hipoalbuminemia es predictor de pancreatitis aguda severa. Objective: To evaluate whether hypoalbuminemia is a predictor of severe acute pancreatitis. Material and method: An analytical, observational, retrospective cohort study was conducted, in which 152 clinical histories of patients with a diagnosis of acute pancreatitis were included, according to the selection criteria they were divided according to the presence or not of severity, it was calculated with the Chi-square statistical test and the relative risk statistic. All the clinical histories included in the research contained the inclusion criteria and belonged to the Gastroenterology service of the Hospital Regional Docente de Trujillo (Regional Teaching Hospital of Trujillo). Results: In a total of 152 patients, the frequency of severity in patients with acute pancreatitis and hypoalbuminemia was 33%; in addition, the frequency of severity in patients with acute pancreatitis without hypoalbuminemia was 1%. It was determined that hypoalbuminemia is a predictor of severity in patients with acute pancreatitis (RR: 25, 95% CI 3.6-45.4, p <0.05). Likewise, within the general characteristics of patients with acute pancreatitis, it was determined that the condition of advanced age (OR 31.4, 95% CI 2.5-54.7, p = 0.022), diabetes mellitus (OR: 31.4, 95% CI 1.6-15. 8, p = 0.027), elevated C reactive protein (OR: 3.89, 95% CI 1.5-7.6, p = 0.016) and hemoconcentration (OR: 147, 95% CI 4- 213.1, p = 0.001), were significantly relevant in the group of patients with severe acute pancreatitis. Conclusion: Hypoalbuminemia is a predictor of severe acute pancreatitis. |
